Fall Sports Physicals 2022
- All Fall sports physicals must be completed and returned to the school by the first day of fall sports which is Monday August 15, 2022.
- Sports physical forms can be found on NPH's website under Forms.
- Physicals will be provided at the High School school for athletes that are unable to have the form completed by their primary care provider.
- School physicals will be held at the High School on Thursday July 28 & Thursday August 4 from 1:30 to 5:30 by appointment only.
- Please fill out the Google Form found by Clicking Here to schedule an appointment.
- If you are having a school physical, be sure to bring the PIAA form with sections 1 to 6 completed.
- Impact testing is required every 2 years. Impact testing can be done remotely. Email Mrs. Silva or Mrs. Sheerer to get a link.
